About Us
The Digital Innovation Hub for Home Hospitalisation has been established to help promote knowledge sharing, collaboration and innovation for all aspects of Home Hospitalisation, the original concept of an innovation hub was an objective of the Interreg NWE CHANCE project which brought together a number of SME’s, Hospitals and Universities to develop a Home Hospitalisation Platform to support Heart Failure.

Background
There are a number of terms used globally to describe the provision of hospital level care for acute conditions which normally require a stay in hospital; Home Hospitalisation, Hospital at Home, Virtual Wards, Hospital with Walls amongst others, all focusing on getting the right care to the right patient at the right time, in the right place.
Home Hospitalisation typically involves;
- Patients treated as though they are being admitted to hospital, but are managed with their own homes.
- Care delivered by multidisciplinary teams including involvement from community based health care initiatives
- Care which is designed to patient and family centred in partnership with the multi-disciplinary teams
- Clearly defined clinical pathways and protocols are put in place

What We Do
The Digital Innovation Hub for Home Hospitalisation aims to:
- Support collaboration and knowledge exchange between industry and healthcare providers in the process of developing and implementing new digital health technologies
- Ensure the continuity and growth of collaborative activity leading to the development of eHealth products to support hospitalization at home.
