Health Improvement Scotland iHub worked with NHS Boards and Health and Social Care Partnerships across Scotland to support the implementation of Hospital at Home services. As part of that work they created the Hospital at Home Toolkit to provide a range of tools and resources to support areas to implement and expand Hospital at Home Services.
